Welcome to our International Memory Clinic (IMC) Patient Hub.

Here you* can:

• Register as a new patient – note that you must be at least 18 years old.

• Book your online First Consultation or Reassessment Consultation using our global appointment system on Semble. The minimum notice for booking is 48 hours.

• Secure your place in the IMC Memory Program.

* You refers to the patient, although we realize that a family-member, friend or POA (Power of Attorney) of the patient may be providing help with registration and the scheduling of consultations on the patient's behalf. During the registration process, government-issued picture ID will have to be uploaded for the patient and, if applicable, the person helping the patient.

New Patients
New patients must register by submitting a "New Patient Registration Form – International Memory Clinic" prior to booking the first consultation. Please follow the steps in the First Consultation section below.

Returning Patients
- If you have been seen in the past four months, but are not a participant in the IMC Memory Program, you can book a reassessment consultation. To book, please click on the light blue button in the Reassessment Consultation section below.
- If you have not been seen in the past four months, you will need to book a first consultation by clicking on the dark blue button in the First Consultation section below.

Securing Your Place in the IMC Memory Program
- If your participation in this program has been approved by your clinician in a first or reassessment consultation within the past four weeks, you can secure your place today – send us an email at info@vejonconsultive.com, requesting your payment link.
- If your first or reassessment consultation was more than four weeks ago and you have not yet secured your place in the program, you will need to be reassessed to ensure the program is built on an accurate picture. Book a reassessment consultation by clicking on the light blue button in the Reassessment Consultation section below.

Cancellation of an Appointment
To cancel an appointment, use the link provided in the confirmatory email sent from info@vejonconsultive.com when your booking was made. The minimum notice for cancellations is 24 hours.
